Community Needs Assessment Survey
The first way for rural communities such as: Colorado City, Snyder, Loraine, Ira, Westbrook, Hermleigh, Roscoe, etc. is to assess what the community needs from those residing in the area. Help assess what the community will benefit from with this survey and we will reward you for participating with a 5% discount redeemable in-store or online. Your discount will be sent to the email provided in the survey and has an expiration date of 1 year from receiving. Each discount is valid for one person per transaction and not redeemable with a discount already present. Redeem by showing your email to the cashier or by inserting a code that will be sent to you separately from admin@mypremierwellness.com.
